Partnerships Leading Care Fellowship

Welcome to the Partnership Leading Care Fellowship cohort of 2026. The Partnerships Leading Care Fellowship is a 12-month structured development programme, where fellows from across the Leeds Health and Care sector have been asked to develop a project within their place of work aligned to one of three neighbourhood health priorities.

Prevention of ill-health, Health Independence & Community Resilience

Supporting people to live well and independently in their communities, while reducing reliance on statutory services.

Health and Wellbeing Innovation, Digital & Policy Translation

Using technology, data and improvement approaches to enhance care, translate policy into practice, and support innovation.

Health Equity, Inclusion & Wellbeing

Promoting equity, inclusion and wellbeing for residents and the health and care workforce across neighbourhood

The project summaries below were collated in May 2026 to encourage collaboration and shared learning across the fellowship cohort. Responsibility for the development and delivery of each project sits with the individual fellows.

We look forward to seeing how these projects progress and evolve over the next 12 months.
